Reflection

Friday .... 

Today i was the incharge of afternoon shift. 
Around 3 pm , boss came and do the rounds so i was unable to attend a new child that just admitted . 
Her diagnosis was viral fever and some AGE also. 
While we are doing rounds near lobby, my colleagues were helping out to set up line and drip for that child and noted her HGT was just 2.9mmol/L . Dr.Richard informed and asked to proceed everything. After we finished the rounds, then we proceed with LP for the child. At first the mother was not keen to give consent for doing LP, but at last she signed for it. Before we did LP, then i was actually wanted to recheck the HGT as my colleagues said from Dr. Aini, suppose to check after half hour of the previous result. While i was wanted to check, then was stopped by Dr.Richard. As i was about to do everything that prepared, i was actually a bit angry, then he explained to me that the result will be the same even though i recheck now cos logically the child didn't take any drinks and food within this half hour so he ordered to bolus 30 mls of D10 for that child then only recheck HGT after done with LP. At first i was just with all my tempered and didn't manage to calm down, proceed with my reports and still have a few new plan at last minute by Dr Law. 
Now, i refresh back everything, i wasn't suppose to be so angry on everyone and really moody this afternoon. just felt breathless. I was suppose to clear up my mind, do everything in order, and i now i can get it over. My colleagues were doing great job helping me all around, really grateful. 
Next time, i should be handle it better. :) Go Go!

Reflection *2*

Date: 1 February 2012
Time: 7am-2pm

A) Second day attachment in ETD...around 11.30am the buzzer sounded 3 times..i was asked by CI to follow in ambulance.. the klien was an unconscious young lady of 21 years old.. at 10th miles Dr.Chiam clinic.. when we arrived, just to see how klien being putting in recovery position and try to wake her up by tapping her shoulder. Later one, SN used the ammonia to wake her up and she finally opened eyes a while then we sent her into ambulance and went to SGH. While in clinic, i can't do anything yet i dont know what should i do.
R: I think i should experience more emergency cases then i shall know what i have to do next while reaching the klien in emergency scenes.

b) While the klien, named Jihan arrived SGH, i tried to help her set IV line. I've tried 2 times on her left hand but failed. Then Dr.Siti tried on her Right hand, both times failed too. She then tried on her left hand again near the spot i've tried to set on, then she managed to get it. indeed that is a good line which first i tried to set.
R: I should practise more and not nervous while being told to set IV alone without CI eyeing on me! I have confident but just the skill not sharp enough yet to handle a simple case.

Reflection *1*

Date: 31 January 2012
Time: 7am -2pm
Ward: Emergency & Trauma

A) We arrive ward around 7am. We all sit in the rest room while waiting for CI Yii to arrive and guide us. That time, JM told us to wait for CI, then we all went into the rest room and wait until 8am. Actually, between 7am -8 am, 2 SN was doing the bed making, and we all just walk away.
R: We should be helping out eventhough we are the first day to arrive this ward. instead of just sit inside rest room and wait for an hour which it was a waste.

B) Today around 11.45 am i was being asked to set IV line! this is the first time i did it! i was a bit nervous but CI said i did well. so lucky and i shall practise more. :) after that, i took the blood spesimen for checking the FBC and also blood sugar. After i checked the blood sugar for the klien, it showed 21.2mmol/L which klien had DKA.
R: I should ask the klien whether he has Diabetic and Insulin injection which i forgot to do so.
